Spoilage Defined

We're having some trouble recording spoilage, some operators call their make ready and bad product in the run as spoilage. I would think make ready is part of setup that can be charged, but any bad product through the run would be spoilage. Is there a defined standard for what spoilage actually is, or would we define spoilage to fit our production? Also, what is industry standard on spoilage, my operations manager says we should be getting down to 1.5% and things i have read on this forum say around 5%. I guess knowing exactly how spoilage is defined will also help know what our goal should be.
